Physician Referrals Made Easy

Thank you for referring your patient to The Heart Group. The physicians at The Heart Group value the relationships we have with our referring physicians and welcome any feedback or comments you may have regarding our practice. We take care to reinforce your role as your patient’s primary care physician and our role as a cardiac consultant.

Clear and appropriate communication with our referring physicians is important to us. If desired, our staff will call you to confirm your patient has made an appointment with one of our physicians. After the patient’s visit, our physician will forward a summary of diagnostic reports and treatments to you as soon as it becomes available.

To facilitate the referral, we will ask for pertinent medical history information. The information will be given to The Heart Group physician you select or to an appropriate physician. Our staff will call your patient, schedule the first appointment and answer any questions the patient may have regarding their visit to our practice.
